What is DG Form, and How is it Prepared?

Appropriate industry protocol must be followed before shipping any hazardous material. The two essential documents required to initiate a DGR shipment are the air waybill and the DG form. The information provided in these documents is critical to the success of the migration. This is a standard procedure and is required for all dangerous goods shipments.

A DG form (Dangerous Goods Declaration) that does not meet the applicable provisions of ICAO’s Safe Carriage of Dangerous Goods by Air and IATA Dangerous Goods Regulations is a direct violation of the law for air transport and is subject to civil or criminal conditions.

According to all dangerous goods regulations (UN TDG, DOT, IATA, IMDG, ADR), it is the responsibility to define the DG form correctly. This critical form must be retained for a certain period of time (3 months to 2 years) depending on the shipping method.

What Information Is Included In The DG Form?

The DG form is vital for the transport of dangerous goods and contains the following information:

  • Sender contact information
  • Airline invoice number
  • Buyer contact information
  • Sender contact information
  • Hazardous substance items, amount
  • Departure airport
  • Destination airport
  • Statement

Other items and information may also be required.

The main purpose of the DG form is for the carrier to provide critical information to the airplane operator or carrier in a consistent format. This standard is part of the IATA Dangerous Goods Regulation (DGR).

What is the DG Form and How is it Prepared?

  • Page Numbers

If the form has more than one page, this number should be numbered consecutively.

  • Shipper & Consignee

It must contain the name and address of the shipper and the consignee.

  • Date

The form must have a date. This may be the date the form was prepared or the date it was first issued to the carrier.

  • Dangerous Goods Part

Each dangerous goods item under the relevant shipment is indicated here and this information should be in a specific order as follows.

  • UN Number
  • Full Shipping Name
  • Class
  • Auxiliary dangerous class
  • Packing group

This information should always be in this order and should never be rearranged while the document is being prepared.

  • Quantity of Dangerous Goods

In this field, you must declare the total number of packages, their type and quantity of dangerous goods. For explosives the quantity is the net weight of the explosive, for other dangerous goods, it is the volume or mass.

  • Limited Quantities

If the submitted shipment is in limited quantities, it should also be stated on the form in the same way.

  • Exceptional Quantities

If the submitted shipment is in exceptional quantities, it should also be stated on the form in the same way.

  • Temperature controlled

Substances requiring temperature control and self-reactive should be specified.

  • Signature

The shipper and packer must sign the document electronically.

  • Declaration or Certificate (Declaration or Certification)

A declaration must be made to certify that the shipment complies with applicable regulations.

  • Retention of DG Form

Both sender and carrier will keep a copy of any additional documents required by the form for at least 3 months.
